Temple Beth Sholom Tribute Form
TBS tribute funds are designed to maintain the programs of the synagogue. There
is a minimum charge of $10.00 per tribute (your donation remains confidential).
Please consider making a tribute to honor or thank someone, to memorialize a
yahrzeit and to wish speedy recovery to someone who is ill.
Please complete the PDF form
and bring or mail it to the synagogue office along with your check made payable
to Temple Beth Sholom. Please make sure you include all the required information.
